“Behavioral Health Services in Primary Care An Essential Component of Integrated Care” (APA)

What is Integrated Care?
The Center for Medicare and Medicaid Services(CMS) currently recognized two primary integrated health-care models -  primary care 
behaviorist model (also called primary care behavioral health, or PCBH) and the collaborative care model (CoCM—care management for patients with mental health conditions. 

Integrated behavioral health care is the collaboration of the primary care provider and a behavioral health specialist teaming up to provide high quality holistic care that benefit the patient, physicians, and the medical community.

BHC is a generalist who sees any behavioral issue and all ages.
 

BHC services are available on a same-day OR by appointment.
 

BHC is a regular member of the team and is ready to help in a variety of ways, such as pre-PCP visits, after-PCP visits, classes, and group medical visits.
 

BHC sees agreed upon number of patients every day.
 

BHC available to teach behavioral interventions to others on the team.

BHC helps the team develop pathways or protocols that may involve BHC help in care for high-impact patient groups.

Primary physician provide holistic treatment by conveniently and easily making mental health and/or substance abuse referrals to the on-site Behavioral Health Care Manager.  Patients with related behavior health concerns finds it more comfortable to seek mental health services in a familiar setting.

The Behavioral Health Manager can also be available to organize and facilitate various other types of health and wellness groups such as:
●Mental Health Matters
●Stress & Well-Being
●Healthy Thinking
●Coping with Stress
●Anger Management
●Diabetes Awareness
●Eating and Living Well

Patient will have a “one-stop” holistic treatment experience by receiving medical and behavioral integrated care at their one (1) scheduled appointment.

As a result of stigma, patients often times feel more comfortable receiving behavioral health treatment in familiar settings rather than going to a Mental Health Counselor office setting.

The Patient can see the on-site Behavioral Health Clinician for stress problem, mental health, substance use, health behavior or all of the above.

Private Matters Counseling Services, LLC(PMCS) goal is to provide the Primary Care Provider the needed mental health support the medical team doesn’t have enough time or training to address as the contract Behavior Health Care Manager.

Additional benefits includes but not limited to:
1. Help the Medical Provider provide “whole-person care.”  

2. Available to provide behavioral health care at the right times in the patient’s medical care treatment. 

3. PMCS is Medicaid approved in North Carolina and South Carolina and meet the criteria for Medicare billing as a Behavioral Health Care Manager.
