“Behavioral Health Services in Primary Care An Essential Component of Integrated Care” (APA)
What is Integrated Care?
The Center for Medicare and Medicaid Services(CMS) currently recognized two primary integrated health-care models - primary care behaviorist model (also called primary care behavioral health, or PCBH) and the collaborative care model (CoCM—care management for patients with mental health conditions.
Integrated behavioral health care is the collaboration of the primary care provider and a behavioral health specialist teaming up to provide high quality holistic care that benefit the patient, physicians, and the medical community.
